What exactly is CareSwaps?
CareSwaps is a healthcare technology platform that uses algorithmic matching to help families find transfer opportunities between skilled nursing facilities. When your loved one needs to move โ closer to family, to better care, or to a specific facility โ our platform searches a network of participating facilities to identify potential matches. We coordinate the timing and paperwork so families can focus on what matters.
How is this different from calling facilities directly?
When you call facilities individually, you're limited to whoever happens to have an open bed at that moment. CareSwaps uses a swap model โ our algorithm identifies situations where two or more facilities can simultaneously exchange residents, creating openings that might not otherwise be visible. We also find multi-facility "swap chains" that create cascading opportunities across the network.
Does CareSwaps choose a facility for my loved one?
No. CareSwaps provides matching options based on operational factors โ bed availability, payer acceptance, geographic proximity, and timing. All clinical decisions about whether a transfer is appropriate are made independently by the licensed clinical staff at each facility. We provide the technology tools; facilities and families make the decisions.
What does the $299/month cover?
Your Platform Access Subscription includes full access to our matching algorithm, swap monitoring dashboard, compliance document tools, and our vetted transport provider directory. This is a technology platform subscription for software access โ not a one-time fee or a per-transfer charge. You can cancel anytime with no cancellation fee.
What if CareSwaps can't find a match?
Transfer outcomes depend on facility availability, clinical appropriateness, and regulatory requirements โ factors outside our control. We can't guarantee a specific transfer or timeline. If you're within 7 days of your first payment, you can cancel for a full refund. After that, you can cancel anytime and receive a prorated refund if no matching queries have been run in the current billing cycle.
Is my family's information safe?
Yes. CareSwaps is HIPAA compliant. All patient data is stored exclusively with BAA-signed service providers (Google Workspace, Jotform, Paubox) and transmitted via encrypted channels. We never store patient health information in non-BAA systems. Your data is protected using HIPAA-compliant security standards, including BAA-signed service providers and encrypted communications.
Does CareSwaps handle transportation?
CareSwaps does not provide, coordinate, dispatch, or arrange transportation. We maintain a directory of vetted and credentialed transportation providers that you can browse. You contract directly with the transport company of your choice โ or you can arrange your own transportation. CareSwaps is a technology platform, not a transportation provider.
What states do you serve?
We're launching in Colorado and building our initial facility network along the Front Range and statewide. Our goal is to expand to Texas and additional states in 2026-2027. The bigger the network, the more matching opportunities the algorithm finds โ so we're growing deliberately.

Transfer outcomes depend on facility availability, clinical appropriateness, and regulatory requirements. CareSwaps does not guarantee any specific transfer, timeline, or outcome. At launch, CareSwaps matching services are available for private-pay and commercially insured residents only. Medicare and Medicaid patients are not currently eligible for platform matching.
